Provider Information
Provider agencies are the backbone of the
long-term care service delivery system. If you are currently a Home
and Community-Based Services (HCBS) or Medicaid-certified provider
serving Adams, Arapahoe, Denver, Douglas, Elbert counties, Longterm
Care Options makes every effort to maintain your information on
file. Please view the Provider List on our website's resource tab
to ensure your organization's information is included. If you would
like to submit information to LTCO, please call our office at

As a Single Entry Point agency, Longterm Care Options offers many
opportunities for community outreach for Medicaid-certified
provider organizations.
Provider List
LTCO maintains a provider list of HCBS and
Medicaid-Certified providers serving Adams, Arapahoe, Denver,
Douglas, Elbert counties. Please see our resource tab for the
most current information.
Provider Fair
LTCO hosts provider fairs so that LTCO case
managers and certified providers have an opportunity to network.
Every provider is invited on an annual basis for a specific date.

Informational Materials Distribution
Please feel free to drop off your organization's
informational materials at anytime to LTCO's receptionist staff.
These materials will be made available for distribution to case
management staff and/or clients.
